Financial Responsibility & Assistance Program

If you have a health insurance HMO/PPO plan or other form of coverage(s) that may cover some or all of the Services to be provided, the Fort Myers Endoscopy Center's billing department will initiate contact with them to determine your cost-sharing responsibilities for the Fort Myers Endoscopy Center's bill. Your cost- sharing responsibility may be in the form of Co-pay, Co-insurance or Deductible. You may contact them directly as well for additional information concerning your cost-sharing responsibilities.
If it is determined that you have cost-sharing responsibilities for the Fort Myers Endoscopy Center services, in accordance with center's financial assistance policies, you will be required to pay your cost-sharing responsibilities in full on or before the date that Services are provided. Accepted forms of payment include cash, personal check, credit card and care credit. If you are unable to pay your cost-sharing responsibilities in full on or before the date that Services are provided, the Fort Myers Endoscopy Centers will gladly assist you with other options like applying for "Care Credit" or a 90-day payment plan.
If you believe you are medically indigent or you are not covered by any health insurance policy or program, then upon request or in its sole discretion, the Fort Myers Endoscopy Center may offer you a discount on the amount due based on the current Federal Poverty Level scale and adjusted accordingly. An application with proof of income and expenses may be required.
After you’re discharged, a statement will be mailed to you for any balance due after your insurance company has processed your claim. Payment is due upon receipt and may be made by any of the methods as mentioned above. If not collected at the time of service or additional cost sharing responsibility is due, the center will produce and send no more than three statements before the balance is sent to collections.

Good Faith Estimate
​
Service bundle information is a non-personalized estimate of costs that may be incurred by the patient for anticipated services, and the actual cost will be based on services actually performed.
You have the right to request a personalized estimate from the center. This estimate must be provided to you within seven (7) days from the receipt of the request by Fort Myers Endoscopy Center. It is important to understand that this is only an estimate, and actual costs may exceed the estimate. It is advised that you contact your insurer or health maintenance organization (HMO) regarding your cost-sharing responsibilities. You may request and obtain a Good Faith Estimate by calling Fort Myers Endoscopy Center at (239) 482-2280

Itemized Bill​
Fort Myers Endoscopy Center will provide an itemized statement or bill upon request of the patient or the patient's survivor or legal guardian. The itemized statement or bill shall be provided within 7 business days after the patient's discharge or request, whichever is later.

​Provider Disclosure
​
Services may be provided in this health care facility by Fort Myers Endoscopy Center as well as by other health care providers who may separately bill the patient for service provided. These service providers may or may not participate with the same health insurers or health maintenance organizations as Fort Myers Endoscopy Center. You may request a more personalized estimate of charges from these other health care providers by contacting the health care providers directly. Fort Myers Endoscopy Center may contract with providers for anesthesiology services and pathology; these services are billed separately from Fort Myers Endoscopy Center. You may contact these providers through their contact information provided below.
